At South Cove Community Health Center (SCCHC), we care about each patient and family member’s experience with our clinical care and service. This includes protecting your privacy.
Protecting your privacy is required by state and local government, regulatory agencies and SCCHC’s own policies — and we believe it is the right thing to do.
When we talk about privacy, we are referring to protected health information (PHI — information about a patient’s injuries, diagnosis, medical history or treatment) and personal information (PI — a patient’s name, address, social security number, etc.). You can read our current Notices of Privacy Practices in detail. This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
